Loose wood banisters

We have oak banisters with pickets every 4" on all three floors. They were not properly installed two years ago and are falling apart. My 7 year old was even struck in the face when a picket upstairs fell out and came crashing down the stairs. I have tried to find books on this type of repair without luck. They are not attatched to the floor joists. I have tried glueing and nailing each picket spacer in place, but the pickets still come out. I don't trust the contractor that installed them to do the repair and no one else will touch them. HELP! Thank you.

i have a cracked base that anchors the railing to the upstairs hall way floor. i need to replace or fix, the length is approx. 13 feet. the base is anchored to the floor with wood screws that have been capped. the crack in the base makes the railing loose.
